Please follow these instructions to ensure the optimal repair of your instruments.
Clean and sterilize all instruments to be repaired.
Use a sturdy shipping box with packing material to protect the instruments.
Include the repair form (located below) and indicate which instruments are being sent, if a quote is requested prior to repair, and any special instructions.
Ship via a secure, trackable method i.e. UPS or FedEx.
Please note: Corza Ophthalmology is not liable for any instruments that are lost or damaged while in transit to our facility.
